Every student must have a new Emergency Contact & Confidential Health
Information Form completed every year. Drop this completed form off to the school
nurse.

Please inform the nurse of any significant health issues for your student. This is
extremely important for the freshman class. Parents of freshmen will be asked to
complete specific forms for this health condition and give the documentation to the
school nurse. (We already have documentation for sophomores, juniors and seniors, but
if there have been any changes in your student’s condition, please inform the nurse.) If
your student carries an inhaler or an Epi Pen on their person, a form needs to be filled out
yearly (no matter the grade).

The Park Hill School District does not provide medication to students. Parents or
guardians who want the nurse to give medications (over the counter or prescription) to
their student will need to provide the medication to the health room and fill out the
appropriate paperwork EACH school year, allowing the school nurse to administer the
medication. Park Hill High School health room asks that you send/provide no more than
30 pills at one time. Another option for your high school student regarding medications
is that they can carry one (1) dose of medication on their person as long as it is in its
original container.

Missouri State law requires that your students have the appropriate immunizations
to attend school. If you choose not to have your child immunized, or their medical
condition requires that they cannot be immunized, parents need to go to the Health
Department or to their personal physician to have exemption cards completed and
provide a copy to the school nurse.


New Immunization Requirements for the 2017/18 School Year

Who: 2018 Senior Class

What: Meningococcal (Menactra) Vaccination


The state of Missouri has made new Immunizations requirements for the 2017/18 school year. This will affect those students who are JUNIORS this year (2016/17). These students will NOT be able to begin their senior year until proof of the Meningococcal (Menactra) Immunization can be provided. Many of the students have had this vaccination before the age of 16, but will need a BOOSTER SHOT before beginning school in the 2017/18 school year. Some students have not had it at all and will need to get the vaccination. Our school nurses sent information to you with your student's midterm grades in March. Be sure you read this information because it will provide you the needed details regarding this vaccination for your student. Our school nurses are hoping you can follow through with providing the required information before the end of this school year to avoid issues and long lines at Registration. If you have any questions or concerns, please do not hesitate to contact one of our school nurses.
